Membership Fees

Membership fees are due on the [first] of each month via the payment method provided during sign-up.

Payment Methods

Accepted forms of payment include credit card or debit card. Payment will be processed automatically every month.

Non-Refundable

Membership fees are non-refundable, except where required by law. Fees are not prorated for partial months of membership.

Automatic Renewal

Memberships renew automatically unless canceled as per the cancellation policy. A minimum of 6 months is required.

Service Frequency

Service benefits are available as per the membership plan and will be scheduled as recommended by the provider.

Unused Services

Any unused benefits do not roll over into the next month or year. Services must be used within the membership term, while the membership is active. Once a membership is terminated unused benefits cannot be utilized.

Scheduling

Services must be scheduled in advance and are subject to availability. Cancellations or reschedules must be made at least 24-48 hours in advance. Failure to cancel within 24-48 hours may result in a charge for the service.

Priority Booking

Members will be accommodated when the preferred provider is in office, an alternate provider will be offered if the preferred provider is not available, or the member may choose a date when the preferred provider is available.

Service Upgrades

Members can upgrade to additional treatments or services for an additional fee, which will be billed separately at the time of service.

Medical and Age Eligibility

Some treatments, such as Botox, may require medical consultation or approval. The practice reserves the right to decline treatment if the individual does not meet eligibility requirements or when treatment is contraindicated. Patients under the age of 21 may not be eligible for some treatments or services, the practice may offer a comparable service as an alternative.

Cancellation by Member

Members may cancel their membership at any time with [30] days' written notice, after the required 6-month minimum membership has been completed. Membership will remain active until the end of the current billing cycle.

Cancellation by Practice

The practice may terminate membership at any time if a member violates the terms and conditions, fails to pay fees, or engages in inappropriate behavior. Any prepaid fees are non-refundable unless required by law.

No Refunds for Unused Services

Membership fees are non-refundable, and unused services are not refundable or transferable.

Health Consultation

Some services, such as injectables (Botox, Fillers), and Morpheus, require an initial consultation and may have specific medical eligibility requirements.

Contraindications

Members must disclose any health conditions, medications, or allergies that may affect their ability to safely receive services. It is the member's responsibility to inform the provider of such conditions.

Pregnancy

Some treatments are contraindicated for pregnant or breastfeeding individuals. Consult with a medical provider before undergoing any treatments if pregnant or breastfeeding.

Age Limits

Patients who are under the age of 21 may not be eligible for all treatments or services. The practice may offer alternative treatments.
